Which insurance automation decisions need human review

Use human review where a task needs interpretation, unresolved-source decisions or authority that preparation alone cannot establish. Define what the reviewer accepts: facts, wording, recipient, artifact edition and proposed action. A useful disposition says what may proceed and what remains open.

Separate factual checks, judgment and action authority

For an agency account team, reviewing a drafted missing-information email is a different decision from interpreting a policy condition or allowing a record update. Check cited facts against their source periods. Assign insurance interpretation to the professional responsible for that decision in your agency; do not treat a preparer’s wording check as their determination.

Keep the reviewer’s decision separate from the acting identity’s permission. Someone may accept a sentence without authorizing a send, approving a new recipient or granting access to a system. Decide the applicable review responsibilities from the actual agency procedure, task and agreements rather than a universal software default.

Four checks for a meaningful review decision

  1. Name the question for the reviewer

    Task owner

Specify whether the request concerns a fact, an interpretation, communication wording or a proposed action. Include the intended recipient and next use, plus the owner responsible for an unresolved decision.

  2. Provide the edition and open evidence

    Preparer and source owner

Attach safe source references, periods, artifact edition and proposed changes. Make unknown facts visible. A complete-looking draft should not hide the missing input the reviewer needs.

  3. Record a scoped disposition

    Task reviewer

State accepted, returned for correction or held for evidence. Name the exact content and action edition covered, remaining conditions and reviewer. Avoid a bare approved label that could be reused for another destination.

  4. Check changes and the receiving scope

    Receiving owner and action owner

Return changed facts, recipients or actions for a relevant new disposition. Have the receiving colleague accept the remaining task. If an action is later allowed, compare its result with a separate destination observation.

Worked dispositions: wording accepted, other decisions open

Illustrative example using synthetic data. R1/D1/S1/F1/W1/I1 and recipient aliases are fictional; no customer message, reviewer credential, configured control or observed action is represented.

R1 · D1 v1 offline email textTone and missing-information question, based on S1 v1 for the requested 2027 term.Wording accepted for internal preparation. Current recipient and send authority remain unverified.Preparer: retain those open conditions with D1 v1.
F1 returned · source period differsAttachment covers the prior 2026 term; the question needs the current requested term.Availability confirmed; current-period fact not verified. Do not replace the unknown with the prior answer.Source owner: obtain the matching period; reviewer: assess the affected fact.
D1 v2 · S1 v2 recipient instructionA new source names recipient alias R-B instead of R-A.R1 does not accept the changed recipient. Revised text awaits current recipient/action disposition.Task reviewer and permission authority: resolve recipient, exact message and allowed action.
W1 proposed record-note actionThe team proposes storing the reviewed wording in system I1.New action held; wording acceptance does not establish a record-write grant or persisted result.Action owner: establish authority and scoped review before attempt; verify any later result separately.

Return the unresolved decision to an accountable owner

A review handoff should retain the reviewer, exact artifact/source references, accepted scope and remaining question. In the example, H1 can be accepted as a task to obtain the current-period fact; that does not close the recipient or record-write branches. Record the receiving owner’s response rather than treating routing as acceptance.

  • Missing reviewer or expertise: assign an appropriate reviewer or backup; leave the affected judgment unresolved.
  • A source changes after acceptance: identify the affected statements and preserve the prior disposition as history.
  • Only part of a result is verified: record that part and the unresolved endpoint before further reliance.

Policy context is separate from configured controls

Decla’s marketing-site Terms section 7.2 assigns customers responsibility for reviewing, verifying and approving generated content before use, submission or reliance. The page displays an August 26, 2026 effective date and a v2.1 footer that refers to superseded April 21 terms. It says a conflicting signed master services agreement (MSA) controls. This is company policy wording, not proof of a task’s deployed review control or a legal determination for your agency.

The Agents Council for Technology (ACT) 2026 Tech Trends landing page recommends written governance and structured role-based training. These support defining responsibilities, not a universal approval rule. Both public bodies were checked October 6, 2026; ACT’s report label does not establish a publication date.

Frequently asked questions

Which insurance automation decisions need human review?

Use human review where a task needs interpretation, unresolved-source decisions or authority that preparation alone cannot establish. Define what the reviewer accepts: facts, wording, recipient, artifact edition and proposed action.

Does using AI create errors and omissions (E&O) risk for an agency?

AI can change where a mistake happens without changing who is responsible for the work. Keep a named reviewer for client-facing output and record what was reviewed. Ask your E&O carrier or advisor how your policy treats AI-assisted work.
